Een aanzienlijk deel van de patiënten met bulleus pemfigoïd ontwikkelt geen blaren. Bij deze patiënten met non-bulleus pemfigoïd kan dit leiden tot een lange vertraging in het stellen van de juiste diagnose. Diagnostische criteria en een optimale strategie zijn daarom nodig voor vroegtijdige herkenning van de ziekte. Wij onderzochten de minimale vereisten voor de diagnose van bulleuze en non-bulleuze varianten van pemfigoïd en de optimale diagnostische strategie.

In deze studie werden verschillende diagnostische testen vergeleken in 1125 opeenvolgende patiënten met een verdenking op pemfigoïd. Gepaard uitgevoerde testen van (1) een huidbiopt voor directe immunofluorescentie (DIF)-microscopie, (2) indi- recte immunofluorescentie op zoutgespleten huid (IIF SSS) in serum, en (3) routine serologische testen ELISA en immunoblot werden vergeleken.
Van de 1125 deelnemers hadden 343 patiënten de diagnose pemfigoïd, met 782 controles. Van de 343 patiënten met pemfigoïd hadden 74 (21,6%, of 1 van 5) non-bulleus pemfigoïd. Een huidbiopt voor DIF was de meest sensitieve diagnostische test en positief bij 88,3% van de patiënten. IIF SSS was minder sensitief (77,0%), maar zeer specifiek (99,9%) en een aanvulling op patiënten met negatieve DIF-resultaten.
De resultaten van de regelmatig gebruikte ELISA voor BP180 NC16A-antistoffen hadden geen toegevoegde diagnostische waarde. Deze bevindingen hebben geleid tot de volgende minimale criteria voor het stellen van de diagnose pemfigoïd (2 uit 3 positief): (1) jeuk en/of predominant cutane blaarvorming, (2) lineare IgG- en/of C3c-deposities (in n-serrated patroon) bij DIF op een huidbiopt, en (3) positieve epidermale aankleuring van IgG met IIF SSS in serum. Deze diagnostische criteria breiden bulleus pemfigoïd uit met de niet-herkende non-bulleuze variant. Een op de vijf patiënten met pemfigoïd presenteerde zich met non-bulleus pemfigoïd, wat een valkuil kan zijn voor de dermatoloog. Zowel een huidbiopt voor DIF als bloedonderzoek met IIF SSS is noodzakelijk voor het stellen van de diagnose van bulleus of non-bulleus pemfigoïd.

Summary

A substantial number of patients with bullous pemphigoid do not develop skin blisters, these patients with nonbullous pemphigoid often have a significant diagnostic delay or may not receive correct diagnosis. Diagnostic criteria and an optimal diagnostic strategy are needed for early recognition. We assessed the minimal requirements for diagnosis of bullous and nonbullous forms of pemphigoid and evaluated the optimal diagnostic strategy. In this paired diagnostic accuracy study 1125 consecutive patients with suspected pemphigoid were analyzed. We compared pairwise performed test of (1) a skin biopsy specimen for the direct immunofluorescence (DIF) microscopy test; (2) indirect immunofluorescence on a human salt-split skin substrate (IIF SSS) test; and (3) routine immunoserologic tests ELISA and immunoblot.

Of the 1125 participants, a total of 343 participants received a pemphigoid diagnosis, with 782 controls. Of the 343 patients with pemphigoid, 74 (21.6%, or 1 in 5) initially presented with nonbullous pemphigoid. The DIF microscopy was the most sensitive diagnostic test and positive in 88.3%, whereas IIF SSS was less sensitive (77.0%) but was highly specific (99.9%) and complemented most cases with negative DIF findings (figure 1). Results of the commonly used BP180 NC16A ELISA did not add diagnostic value for initial diagnosis. These findings lead to the proposed minimal criteria for diagnosing pemphigoid (2 out of 3): (1) pruritus and/or predominant cutaneous blisters,

(2) linear IgG and/or C3c deposits (in an n-serrated pattern) by DIF on a skin biopsy specimen, and (3) positive epidermal side staining of IgG by IIF SSS on a serum sample. These diagnostic criteria extend bullous pemphigoid with the unrecognized nonbullous form. One in five patients with pemphigoid initially presented as nonbullous pemphigoid and, which can be a pitfall for clinicians. Both a skin biopsy for DIF and blood testing (IIF SSS test) should be performed for diagnosis of the bullous and nonbullous variants of pemphigoid.
